How to Remove Blood Stains From Fabric and Mattresses
Cold water only β that's the single most important rule for blood stains. Here's the complete method that actually works for blood on fabric, sheets and mattresses, including older set-in stains.
Blood stains have a specific chemistry that makes them genuinely treatable if you follow the right process β and genuinely set permanently if you don't. The single most critical rule, which trips up most people instinctively reaching for the easiest solution: never use hot water on a blood stain. Heat cooks the proteins in blood (the same way heat cooks an egg), bonding them irreversibly to fabric fibres.
This guide covers the correct cold-water-based method for fresh and set blood stains across fabric and mattresses.
The Science: Why Cold Water Matters
Blood contains haemoglobin and other proteins. At cold to lukewarm temperatures, these proteins remain in a state that can be dissolved and lifted by water and mild detergent action. Once exposed to heat β even moderately warm water, let alone hot water β these proteins denature (a permanent structural change) and bond to fabric fibres in a way that becomes very difficult, often impossible, to fully reverse.
This is the same principle that applies to other protein-based stains (egg, dairy) but is particularly important with blood given how visually obvious and common the staining is.
Fresh Blood Stains: The Standard Method
On Washable Fabric (Clothing, Linens)

Rinse immediately with cold water, running the stained area under a cold tap from the back of the fabric (pushing the blood out rather than further into the fibres). This alone often removes a significant portion of a very fresh stain.

Apply a small amount of liquid laundry detergent or dish soap directly to the stain and gently work it in with your fingers.

Soak in cold water for 15β30 minutes. For more stubborn or slightly older stains, extend the soak to several hours, changing the water if it becomes significantly discoloured.

Check the stain before laundering. If it's substantially reduced or removed, proceed to a normal cold or cool wash cycle. If staining remains, repeat the detergent treatment before washing.

Never put a stained item in the dryer until the stain is fully confirmed gone. As with virtually every stain type covered in this blog, dryer heat permanently sets any remaining protein residue.
On Mattresses
Since mattresses can't be submerged or machine washed, the approach differs:
- Blot (don't rub) excess blood with a clean cloth or paper towel immediately, using cold water to dampen the blotting cloth.
- Mix a solution of cold water with a small amount of dish soap.
- Apply the solution to the stain with a clean cloth, working from the outside of the stain inward to prevent spreading.
- Blot repeatedly with a dry cloth to lift the dissolved stain, alternating between applying the soap solution and blotting dry.
- Once the stain is significantly reduced, rinse by blotting with a cloth dampened with clean cold water (to remove soap residue).
- Blot dry thoroughly, then allow to air dry completely β placing a fan nearby or opening windows speeds this and is important given mattresses don't dry quickly and shouldn't be slept on while damp (mould risk).
Set or Older Blood Stains
Stains that have already dried and been through the heat-setting process (a dryer cycle, or simply time and ambient warmth) require more aggressive treatment, though complete removal isn't always achievable depending on how thoroughly the protein has bonded.
Hydrogen Peroxide Method
3% hydrogen peroxide (from any pharmacy) is genuinely effective on set blood stains due to its oxidising action on the protein structure.
- Test on an inconspicuous area first β hydrogen peroxide can lighten some dyed fabrics.
- Apply directly to the dried stain. You'll typically see fizzing β this is the oxidising reaction actively breaking down the blood proteins.
- Leave for several minutes while the fizzing continues, then blot with a clean cloth.
- Reapply and repeat the process several times for stubborn stains β multiple applications are often more effective than one extended application.
- Rinse with cold water and launder as normal (for washable fabric) or blot dry thoroughly (for mattresses and non-washable items).
Salt Paste Method (Gentler Alternative)
For delicate fabrics where hydrogen peroxide risks colour damage:
- Mix table salt with a small amount of cold water to form a paste.
- Apply to the stain and rub gently.
- Leave for 30 minutes.
- Rinse with cold water.
- Repeat if needed before laundering.
Enzyme-Based Stain Remover
Enzyme cleaners (the same category covered in our pet stain guide and post-illness guide) are specifically effective on protein-based stains including blood.
- Apply the enzyme product according to package instructions.
- Allow the specified dwell time β enzyme products need time for the biological breakdown process to work, often 15β30 minutes or longer for set stains.
- Launder or blot as appropriate for the item.
Meat Tenderiser Method (A Traditional Approach)
An older but genuinely effective method given that meat tenderiser contains enzymes (typically papain or bromelain) specifically designed to break down protein.
- Make a paste of unseasoned meat tenderiser and cold water.
- Apply to the stain and leave for 30β60 minutes.
- Rinse thoroughly with cold water.
- Launder as normal.
This works on the same enzymatic principle as commercial enzyme stain removers and is a useful household-staple alternative if you don't have a dedicated enzyme cleaner on hand.
Special Considerations by Fabric Type
Delicate fabrics (silk, wool): Avoid hydrogen peroxide and harsh scrubbing. Cold water and a very mild soap solution, with gentle blotting rather than rubbing, is the safer approach. For valuable delicate items with significant staining, professional dry cleaning with disclosure of the stain type to the cleaner is advisable.
Dark or coloured fabrics: Hydrogen peroxide carries more risk of visible lightening on coloured fabric. Test in an inconspicuous area first, or favour the enzyme cleaner or salt paste methods, which carry less colourfastness risk.
White fabric: The most forgiving for treatment β hydrogen peroxide and even diluted bleach (as a last resort on robust white cotton, after other methods, and only on fabric confirmed bleach-safe) can be used more aggressively than on coloured items.
Why Complete Removal Isn't Always Possible
Being honest about realistic expectations: a blood stain that has been heat-set (washed and dried before treatment, or exposed to significant heat) may not come out completely regardless of method used. The protein bonding that occurs with heat exposure is, in many cases, a genuinely permanent chemical change to the fabric fibre structure.
This is why the cold-water-first principle matters so much β prevention of heat-setting is far more reliable than any treatment of an already heat-set stain. If you discover a blood stain that's already been through a hot wash or dryer cycle, treatment is still worth attempting (often achieving significant, if not complete, improvement), but expectations should be calibrated accordingly.
Period Stains: The Same Principles Apply
